The social determinants of health — factors such as food, housing, access to healthcare, education and employment — account for 80% of our health status. Yet, many people in our communities struggle with these most basic needs.
The Center for Community Health Improvement (CCHI) at Mass General focuses on these social determinants to eliminate health inequities based on socioeconomic status, race and ethnicity with proven strategies including prevention, early intervention and treatment. In collaboration with our community partners, we are having a measurable impact in Charlestown, Chelsea and Revere.
